Policies

Fees, Billing and Collection

You are responsible for your portion of the fee at the time service is rendered. If you have insurance coverage under one of the managed care, HMO, or specialty health plans for which I am a preferred provider, you are responsible for the applicable co-pay and any deductibles according to your plan. If necessary, you are responsible for obtaining any pre-authorization for services as required by your insurance plan.

Cancellation Policy
When you make an appointment I reserve the whole hour for you.  Please give at least 24-hour notice for cancellation of a session. This is to allow the time to offer the appointment to another client who may be on a cancellation list waiting for an appointment. The full fee will be charged for non-attendance or less than 24 hour notice of cancellation. As I am unable to bill insurance companies for missed appointments, this charge would be your responsibility.

If you are unable to keep an appointment because of sudden illness or an unexpected personal emergency, please contact me as soon as possible.
